Stock Transfer Form

Your charitable deduction will be equal to the full market value of the stock. Transferring stock to Paz Wellness is simple. Please call 435.535.1371, email esterlee@pazwellness.org, or follow the instructions below.

How to Transfer Gifts of Securities

• Please notify Paz Wellness by telephone or email that you will be making a gift by contacting us at 435.535.1371 or esterlee@pazwellness.org. When making a transfer, your name will not accompany the electronic transfer of shares.  We kindly ask that you notify our staff of your intent so we can acknowledge your gift correctly.
• After you have alerted us to expect your gift, please instruct your broker to make the transfer to our broker:
